B64-461. MULTIBLADE SCREWDRIVER. Hultberg-Johanson "Senior" IOB. Patented in 1913 by Peter Johanson of Warren, PA, and manufactured by the Hultberg-Johanson Co. Consists of a 7" handle and five bits with a friction fit. The catch was that you need a separate tool (included) to push the bits out of the handle , which may explain why it wasn't a big seller.
